ABSTRACT

Introducción. Los tumores neuroendocrinos de páncreas son relativamente raros y heterogéneos. Sin embargo, su incidencia se ha incrementado a nivel mundial, y los avances en el diagnóstico y el tratamiento han mejorado la supervivencia. Tienen un pronóstico más favorable que el adenocarcinoma de páncreas, pero el reconocimiento y el abordaje diagnóstico son complejos y requieren un equipo humano multidisciplinario entrenado. Objetivo. Actualizar al médico en el abordaje clínico, patológico, imaginológico y genético, y en la evaluación hormonal basada en la evidencia disponible, brindando herramientas y recomendaciones específicas para las diferentes circunstancias clínicas. Conclusión. La incidencia de los tumores neuroendocrinos de páncreas en los últimos 40 años ha aumentado en más del 600 %, y corresponden a la segunda neoplasia pancreática con gran mortalidad. Actualmente, disponemos de múltiples biomarcadores para caracterizarlos y plantear un tratamiento más personalizado


Background: Pancreatic Neuroendocrine Tumors (nNET) are rare and heterogeneous. However, the incidence has increased worldwide, and the newer diagnostic methods and treatment have improved survival. They have a more favorable prognosis than pancreatic adenocarcinoma, but recognition and overall diagnostic methods are complex and require a trained multidisciplinary team. Aim: To update the clinical, pathological, imaging, genetic and hormonal evaluation based on the available evidence. To provide tools and recommendations for different clinical scenarios. Coclusions: The incidence of pNET in the last 40 years has increased by more than 600% and corresponds to the second pancreatic neoplasia with a high mortality rate

Introducción: dentro de todo proceso de rediseño curricular se debe considerar la percepción que tienen los estudiantes de su curriculum para efectuar futuros cambios y mejoras. Objetivo: comparar la evaluación que los estudiantes de los distintos cursos de Fonoaudiología realizan del currículum. Métodos: estudio cuantitativo, no experimental y de corte transversal. Fueron elegidos 220 estudiantes de primero a quinto año, por muestreo probabilístico por cuotas, encuestados con el cuestionario de evaluación del curriculum. Los datos fueron analizados por el paquete estadístico STATA 11.0 S.E. Resultados: la pertinencia del curriculum y metodologías de enseñanza y evaluación fueron evaluadas mejor por primer año que los otros cuatro grupos. La distribución del tiempo fue mejor evaluada por los estudiantes de primer año en relación a los de cuarto. Los objetivos de la asignatura, fueron mejor evaluados por los de primer año que por los de tercero. En el factor objetivos de la carrera, los estudiantes de primero realizaron una evaluación más positiva que los de segundo, tercero y cuarto año. La distribución de actividades, fue mejor evaluada por los alumnos de primer año que los de cuarto y quinto. En el factor equipo docente, los alumnos de quinto año tuvieron una opinión más negativa que los de primer y tercer año. El cumplimiento de objetivos, fue mejor evaluado por los alumnos de quinto año que los otros cuatro niveles. Conclusiones: al avanzar en el plan de estudios empeora la evaluación del curriculum. Los resultados respaldan la necesidad de efectuar cambios y mejoras en el currículum(AU)

Objective: Compare the curriculum evaluation by the students of different Phonoaudiology courses. Methods: Quantitative, nonexperimental and cross-sectional study. 220 students of the academic years from first to fifth were chosen through probability quota sampling and conducted a survey with the curriculum evaluation questionnaire. The data were analyzed with the statistical package STATA 11.0 S.E. Results: The pertinence of the curriculum and teaching and evaluation methodologies were assessed better by the first year students than what they were by the rest of the students. Time distribution was assessed better by the first year students than the rest. The subject's objectives were assessed better by the first year students that the third year. In the factor major's objectives, the first year students made a more positive evaluation than the second, third and fourth year students. Activities distribution was better assessed by the first year students than fourth and fifth. The factor teaching staff, the fifth year students had a more negative opinion than first and third years. The fulfillment of the objectives was assessed better by the fifth year students that the rest of the four levels. Conclusions: The more advance in the curriculum, the worst the curriculum assessment. The results support the need for making changes and improvements in the curriculum(AU)

ABSTRACT

La nefropatía asociada a las gammapatías monoclonales es debida principalmente al depósito de cadenas ligeras. Las enfermedades renales paraproteinémicas son lesiones asociadas con depósito de inmunoglobulinas intactas o fragmentos de inmunoglobulinas (cadenas pesadas y cadenas ligeras). La enfermedad por depósito de cadenas ligeras es una condición rara, caracterizada por el depósito de cadenas ligeras monoclonales en muchos órganos y a nivel renal predominantemente en glomérulos y membranas basales tubulares. La enfermedad está frecuentemente asociada con desórdenes linfoproliferativos, y la mayoría de casos son causados por depósito de cadenas ligeras kappa. Aunque se presenta sobre todo en cuadros malignos, en ocasiones no se detecta patología hematológica y se denomina idiopática o "primaria". Suele manifestarse como una insuficiencia renal severa con proteinuria nefrótica, no tiene tratamiento claramente establecido y el pronóstico es malo. Se describen las características clínicas e histológicas del segundo caso informado en Colombia de nefropatía por depósito de cadenas ligeras diagnosticado en el contexto de una enfermedad renal paraproteinémica sin datos de malignidad. Nephropathy associated with monoclonal gammopathies is mainly due to light chain deposition. The paraproteinemic kidney diseases are lesions associated with deposition of intact immunoglobulins or fragments of immunoglobulins (heavy and light chains). The disease due to deposition of light chains is a rare condition characterized by deposition of monoclonal light chains in many organs and as for the kidney, predominantly in glomeruli and tubular basement membranes. The disease is frequently associated with lymphoproliferative disorders and the majority of cases are caused by deposition of kappa light chains. Although presented primarily in clinical pictures of malignancy, sometimes no hematological pathology is detected and is called idiopathic or "primary". It usually manifests as severe renal failure with nephrotic proteinuria, has not a clearly established treatment and the prognosis is poor. The clinical and histological features of the second case reported in Colombia of a light chain deposition nephropathy diagnosed in the context of a kidney paraproteinemic disease without malignancy data, is presented. ABSTRACT

Parasitic myomas are rare and their ethiopathogenesis is uncertain. They may develop from a detached fibroid adhering to an extrauterine surface in order to obtain new blood supply. It has been stated that they form from uterine or myoma fragments left behind after morcellation in the abdominopelvic cavity and thus are called "iatrogenic". Surgeons must be aware of this recently reported complication related to the increasing number of laparoscopic procedures. Thorough inspection and washing of the abdominal cavity are recommended. A case of a patient with iatrogenic parasitic myomas, which appeared six years after a laparoscopic supracervical hysterectomy involving a morcellator, is reported.

ABSTRACT

En el diagnóstico rutinario por biopsia renal encontramos enfermedades glomerulares que tienen depósitos estructurales fibrilares en la matriz extracelular del glomérulo, visibles por microscopía electrónica. La amiloidosis es la glomerulopatía más común con depósitos de estas características ultraestructurales. Sin embargo, hay otras glomerulopatías con depósitos similares, negativas con rojo Congo, pero positivas para inmunoglobulinas. La glomerulonefritis fibrilar es una entidad diagnóstica ampliamente reconocida e identificada en, aproximadamente, 0,5% a 1% de las biopsias de riñones nativos. Suele manifestarse por una insuficiencia renal progresiva con síndrome nefrótico-nefrítico e hipertensión arterial sistémica, no tiene tratamiento claramente establecido y su pronóstico es malo. Se describen las características clínicas e histológicas del primer caso de glomerulonefritis fibrilar idiopática informado en Colombia.
